Medical Records

The Medical Records Department is responsible for the integrity, availability and confidentiality of medical records, in accordance with the professional practices and laws in effect.

 

According to the Act Respecting Health Services and Social Services: 
- Every user 14 years of age or over has the right of access to his or her record under certain conditions (section 17)
- The heirs, legatees by particular title and legal representatives of a deceased user are entitled to be given communication of information contained in his or her record to the extent that such communication is necessary for the exercise of their rights in such capacity (section 23)
- The spouse, ascendants or direct descendants of a deceased user are entitled to be given communication of information relating to the cause of death of the user (section 23)

 

How to make a request to obtain copies of your medical record:

 1- For copies of a CLSC record:

Go directly to the Reception area of the CLSC involved between 8 a.m. and 8 p.m. to sign the request.

or

Send the following information or a request by mail or fax:
- Your name
- Your date of birth
- Your health insurance number
- Your phone number
- The name and address of the person you want to send the information to   
- Documents required and the approximate date of care

 2- For copies of a medical record from the Hospital:
Go directly to the Reception area of the Medical Records Department (Room 0270) between 8 a.m. and 4 p.m. (closed from noon to 1 p.m.), from Monday to Friday to sign the request.

or

Send a request with the following information:
- Your name
- Your date of birth
- Your health insurance number
- Your phone number
- Name and address of the person you want to send the information to
- Documents required and approximate date of care
