Local Service Quality and Complaints Commissioner

If you are dissatisfied or require assistance, please first contact the person responsible for the Department from which you received care.  If you are still dissatisfied, you can address a complaint to the local Service Quality and Complaints Commissioner. In addition, if you need help in filing your complaint, the local Service Quality and Complaints Commissioner can assist you or refer you to an organization that has been commissioned by the minister to do so. 

 

An oral or written complaint may be filed.

 

If you want to file an oral complaint, you can:
- call the Commissioner at 514- 630-2225, extension 5277;
- meet the Commissioner in room 1289, located at the front entrance of the Lakeshore General Hospital  (160 Stillview Avenue, Pointe-Claire, H9R 2Y2)


If you want to file a written complaint, you can fill out a form and send it to the Commissioner by:
